Not brainstorming & writing a plan
Not checking spelling, grammar, punctuation etc.
Not regularly reading enough from reliable native-speaking sources

Not practising enough to improve fluency
Not knowing the format before the exam
Not recording and practicing with a mobile phone
Not using a wide enough range of expressions
Not regularly listening enough from reliable native-speaking sources
Worrying too much about making mistakes
